What You Will Experience

When diseases like cancer and cardiac or neurologic conditions have reached advanced stages, hospice care focuses on managing symptoms and enhancing comfort.

Our skillful hospice and palliative medicine physicians and clinicians are available 24/7 for questions, support and home visits, as needed. They will provide practical advice, emotional and spiritual support, and personalized problem-solving.

Hospice How You Need It

We provide four primary levels of care and support for patients who have a prognosis of less than six months to live:

Routine Care

Intermittent care provided in your home or assisted living facility, as well as support for your family members and care partners.

Respite Care

Admission to a skilled nursing facility for five preplanned days, to give your family members and care partners an opportunity for rejuvenation while your care needs are being met.

General Inpatient Care

Acute symptom management at OhioHealth Kobacker House or one of our hospitals.

Continuous Care

Acute symptom management in your home.

If hospice is requested, an OhioHealth at Home hospice provider will visit your home and explain how we can best help you and your family. We also coordinate with your physicians to develop the best care plan for your needs.
